Let's look at the details.
1) Synthesizing combines 2 pieces of equipment of the same category to create an improved item.
2) The item you choose first is the base item and thus determines what the item becomes.
3) After an item is synthesized it gains a green +Number, this number is limited by the rank of the item.
- Rank 1 items max at +9
- Rank 2 items max at +19
- Rank 3 items max at +29
- Rank 4 items max at +39
- Rank 5 items max at +49
- Rank 6 items and Dengeki/Boss items max at +99!
4) Rank of items determines how much condition boost they recieve, e.g Rank 1 recieves 1 per synthesis level, Rank 6 recieves 6 per synthesis level.
5) Synthesizing an item with another synthesized item will give a condition boost equal to that item's synthesis level, e.g If I choose a Dengeki Item as a base 'Box' and I synthesize it with a Rank 6 item which has a synthesis level of 99, which equates to (99 x 6) a condition bonus of 594%!!! So I will end up with 'Box +99' with a condition of 694%!!!
6) In order to get a max condition of 694% you must choose a Dengeki/Boss or rank 6 item which has not been previously synthesized or has only been synthesized with Rank 6 items.
7) Condition on an item is a multiplier for that item's base stats, e.g a Laser Blade with 694% Condition will be 6.94 times as powerful with a Satk aptitude of 347%!!!!!
Negative aptitudes also multiply, hence avoid synthing with items which have negative aptitudes.
9) Rare metals can be synthesized which will increase the base aptitude by 1% of its relative stat. These use up 1 point of Synthesis level on an item.
10) You can assign skills and specials onto your synthesis weapon provided that there are enough points for the skill allocation and that the skill isn't exclusive to an item.

Step 1: Get to level 8 in story mode and go to the boss level, steal Noire's 6 star Skull Mask.
Step 2: Farm lots of head-gear for synthesizing, I reccommend doing Index Dungeon cloning the zombies for lots of 2 star.
Step 3: Synthesize the Skull Mask to have whatever skills you desire, Let's give it No Pain No Gain and the special move Bone Cold which comes with the Skull Mask, so Synthesize Skull Mask with Appetite Engine.
Step 4: OPTIONAL - Synthesize your desired outcome Headwear with Rare metals to either gain or increase the Headwear's base aptitudes. E.G I will use Lucilia Ornament and fuse it with a rare metal which increases Satk by 1%.
Step 5: Synthesize all the Headwear you collected, first on eachother i.e Robo Masks & Curse Bands to their limit, if you collected the 2 star ones from Index Dungeon they limit at +19.
Step 6: Synthesize all the synthesized (+19 Curse Bands/ Robo Masks) onto the 6 Star Skull Mask (it's cheaper this way than synthesizing all the headgear directly onto the Skull Mask 1 by 1), Remember to always choose the 6 Star Skull Mask as the base and to always re-select the skills you want, Keep synthesizing til you reach +98 (Don't need the extra +1 because it's redundant on the final synthesis).
Step 7: This is the final Synthesis for your Uber Headwear. Synthesize the +99 Skull Mask on your desired Dengeki/Boss/6* Headwear, Remember to re-select skills. Profit!
In my example I used a Lucilia Ornament with 1 time synthesis with a Rare-Metal which increases atk. The resultant is :
Condition: 689% (I lose 5 % because I used one level of synthesis for the rare Metal)
SATK: 6% (Originally 1% from synthing with Rare Metal)
SDF: 137% (Originally 20%)
LDF: 137% (Originally 20%)
LAT: 137% (Originally 20%)
Now the reason why I synthesized with a Rare Metal is because of the boost you recieve from Mod Parts, You will recieve a percentage increase provided that the item has at least 1% gain in a stat. However for each Rare Metal synthesis you forego the extra 5% condition you would have recieved from just synthing soley with 6* items.
Now lets say we have an L - Head Mod chip that we've boosted to 255%, let's add that onto our Lucilia Ornament.
The final result is:
Lucilia Ornament+99
Skills: No Pain No Gain, Bone Cold
Condition: 689%
SATK: 261% (Lucky I synthed in that Rare Metal or else this would be zero)
SDF: 392% (Originally 20%)
LDF: 392% (Originally 20%)
LAT: 392% (Originally 20%)
(The extra 255% isn't visible when you look at the item, but it's noticeable when you look at your stats)
Ideally you can synth rare-metals for all missing stats or just to massively boost 1 stat of your choice.

Tip: For synthesizing swords/axes/staves/spears the best place to get fodder items (Items you collect just to increase the synthesis level) is level 8, there will always be at least 2 Hook shots per level, and it's always in the room you start in, so just dive and collect, rinse and repeat and you will have spiffy uber Swords/Axes/Staves/Spears in no time. However getting 6* items in the first place to fuse with is the tricky part, You can always steal a 6 star Sword off Laharl however it requires trekking all the way to the bottom of the dungeon. Please list if you know of any ways to gather 6* items sooner.
